DetailsNoted that 9 registered high risk pregnant mothers in last quarter (January to March 2025) was having 0% birth outcomes. It is important to register high risk pregnant mothers; Identify, Retain and Respond (IRR) Un documented birth cohorts does not inform the health facility where these mothers are and what happened to them, hence no accountability of reduction of MMR In the community by MOH. Suggested solutions to the above problem: -Assigning a midwife to follow up the high risk pregnant mothers birth outcomes -Airtime consideration for calling mothers to enquire about what happened -Attach a nearby VHT to a high risk mother -Opening up a CQI project with the goal of documenting the outcomes of all high risk mothers Set objective of this to improve from 0% documented birth outcomes of high risk mothers in April to 60% documentation by December 2025 Plot a graph using results from data captured Identify tested change that brings about improvement and put it on the positive trend on the graph. The same applies to a decline; show the cause on the same graph. Note: Staff meetings would come up with the likely causes of the gaps present if at all data was collected, analyzed and reported the situation. Hence meetings of MPDSR will come up with the well informed decisions that prevent maternal and prenatal deaths. It’s un wise to wait for death and then remember to all for an MPDSR meeting to review the case.
